TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, ENTRANTS WAIVE THE ABILITY TO BRING CLAIMS IN A CLASS ACTION FORMAT.

BY ENTERING THE PROMOTION, YOU AGREE TO THESE OFFICIAL RULES, WHICH ARE A BINDING CONTRACT, SO READ THEM CAREFULLY BEFORE ENTERING. TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THIS CONTRACT INCLUDES RELEASES AND INDEMNITIES IN FAVOR OF THE PROMOTION PARTIES AND A LIMITATION OF YOUR RIGHTS AND REMEDIES.

1. Eligibility

The Absolute Aesthetics Win Botox for a Year Sweepstakes (the “Promotion” or “Sweepstakes”) is open only to legal residents of the states of Texas and Oklahoma (the “Territory”) who are at least eighteen (18) years of age as of the date of entry. Employees, officers, owners and directors of Absolute Aesthetics LLC (“Sponsor”) and its respective parent companies, affiliates, subsidiaries, and advertising, promotion, fulfillment and marketing agencies (collectively, the “Promotion Parties”), their immediate family members, and those living in the same household as such individuals (whether legally related or not), are not eligible to enter or win. “Immediate family members” means spouse, partner, parents, legal guardians, in-laws, grandparents, siblings, children and grandchildren, and their respective spouses. “Living in the same household” means sharing the same residence at least three (3) months out of the year.

The Promotion is void outside the Territory and void where prohibited by law. All applicable federal, state and local laws and regulations apply.

By participating, you agree to these Official Rules and to the decisions of Sponsor, including its interpretation of these Official Rules, administration of the Promotion and selection of the winner, which are final and binding in all respects, to the extent permitted by applicable law.

2. Promotion Period

The Promotion begins at 9:00 a.m. United States Central Time (“CT”) on Friday, November 20, 2026 and ends at 11:59 p.m. CT on Thursday, December 24, 2026 (the “Promotion Period”). Sponsor’s designated computer clock and Square’s order timestamps are the official timekeeping devices for the Promotion.

3. How to Enter

During the Promotion Period there are two ways to enter. Both are described below.

A. Purchase Entries. During the Promotion Period you will receive one (1) entry for every full one hundred dollars ($100 USD) you pay for any of the following purchases made through the Absolute Aesthetics online store or online checkout at https://absoluteaesthetics.org/ (each, a “Qualifying Purchase”):

- a Black Friday treatment pack; and

- an Absolute Aesthetics gift card.

Entries are calculated on the amount you actually pay for each order, before sales tax, tips and shipping, after discounts, and rounded down to the nearest $100. Amounts under $100 on a single order do not earn an entry. For example, a $950 pack purchase earns nine (9) entries, a $525 pack earns five (5) entries, and a $100 gift card purchase earns one (1) entry.

The following do not earn entries: regular in-clinic services and treatments; in-clinic skincare or retail products; any bonus value added to a gift card (you earn entries on the amount paid, not the amount the card is worth); the use or redemption of a gift card to pay for anything; and any purchase not made through the channels listed above. If a Qualifying Purchase is refunded, canceled or charged back, the entries associated with that purchase are void.

There is no limit on the number of Qualifying Purchases you may make, but total entries are subject to the limit in Section 4.

B. Free Alternate Method of Entry (No Purchase Necessary). To enter without making a purchase, legibly hand-print your full name, full street address (no P.O. Boxes), email address, phone number and date of birth on a plain 3 x 5 inch piece of paper (a “Mail-In Entry”) and mail it to:

No printable form is provided or required. All of the information listed above must be included for the Mail-In Entry to be valid.

Each Mail-In Entry must be mailed separately in its own hand-addressed, stamped envelope. Photocopied, mechanically reproduced or bulk-mailed entries will not be accepted. Mail-In Entries must be postmarked no later than December 24, 2026 and received no later than December 31, 2026 to be eligible. You will receive one (1) entry for each valid Mail-In Entry received. Mail-In Entries without a legible return address are incomplete and invalid. All Mail-In Entries become the property of Sponsor and will not be returned. Proof of mailing does not constitute proof of receipt. Sponsor is not responsible for postage-due, late, lost, misdirected, incomplete or illegible entries.

4. Entry Limit and Entry Rules

Regardless of method of entry, there is a limit of thirty (30) total entries per person for the entire Promotion, counting Purchase Entries and Mail-In Entries together. Entries above the limit are void.

Attempts to obtain more than the permitted number of entries by using multiple or false names, email addresses, phone numbers, accounts or any other method may result in disqualification. Entries generated by script, macro, robotic or other automated means are void. Entries that are incomplete, illegible, forged, false, late, lost, damaged, misdirected, deceptive or otherwise not in compliance with these Official Rules may be disqualified at Sponsor’s discretion. Entries from persons who do not meet the eligibility requirements are void.

Entrants may not share an email address or telephone number. If there is a dispute over who submitted an entry, the entry will be deemed submitted by the person whose name is on the order or Mail-In Entry, provided that person is eligible. Any potential winner may be required to show proof of identity. Entries are not transferable.

Entry Numbers. Every entry receives its own entry number, like a numbered raffle ticket. For Purchase Entries, the entry number is your order number from your order confirmation or receipt, followed by a dash and a sequence number. For example, if order 1042 earns five (5) entries, those entries are numbered 1042-1 through 1042-5. Each Mail-In Entry is assigned its own entry number, beginning with “M,” when it is logged. Sponsor keeps the official list of all entry numbers, and Sponsor’s records are final. If you cannot find your order number or want to know the entry number for a Mail-In Entry, email [email protected]. Only the first thirty (30) entries per person are numbered and counted. Entry numbers are used to identify entries and to select the winner.

5. Drawing

On or before Friday, January 29, 2027, Sponsor will conduct a random drawing at its offices at 327 W Main St, Denison, Texas, to select one (1) potential winner from among all eligible entries received in accordance with these Official Rules. Each entry has an equal chance of being selected. Purchase Entries and Mail-In Entries are placed in the same drawing.

6. Odds

Odds of winning depend on the total number of eligible entries received.

7. Winner Notification and Verification

Within a commercially reasonable time after the drawing, Sponsor will contact the potential winner using the email address and phone number provided with the entry or order. Sponsor will make at least five (5) attempts to reach the potential winner by email and phone over a period of fifteen (15) days beginning with the first attempt. The potential winner must respond and accept the prize within those fifteen (15) days as a condition of receiving it.

Only one (1) potential winner will be selected, and no alternate winners will be drawn. If the potential winner cannot be reached or does not respond within the fifteen (15) days, declines the prize, is ineligible, or otherwise fails to comply with these Official Rules, the potential winner forfeits the prize and the prize will not be awarded.

After accepting, the winner must sign a declaration of eligibility, release of liability and prize acceptance form (the “Declaration”) and provide the tax information described in Section 10 in person at the first appointment, before any treatment begins. A potential winner becomes a “winner” only after Sponsor verifies eligibility and receives the signed Declaration and tax information.

The Promotion Parties are not responsible for incorrect or illegible contact information, or for emails or calls that are undeliverable because of filtering, full inboxes or any other reason. Notification is deemed to have occurred when the email is sent or the call or voicemail is placed.

Winner Privacy. Sponsor contacts the potential winner directly and does not publish the names of entrants or winners. Any announcement of the winner’s name, photo or other identifying information is made only if the winner chooses, as described in Section 11.

8. Prize

One (1) Grand Prize will be awarded: Win Botox for a Year, consisting of four (4) appointments for Botox treatment at Absolute Aesthetics in Denison, Texas, of up to fifty (50) units each, for up to two hundred (200) units in total. The approximate retail value (“ARV”) of the prize is $2,400, based on Sponsor’s regular price of $12 per unit.

Prize terms:

- Appointments must be scheduled with Sponsor, are subject to availability, and are generally spaced as clinically recommended not more often than every twelve (12) weeks.

- The first appointment must be scheduled within sixty (60) days of the winner’s acceptance. All four appointments must be completed within twelve (12) months of the first appointment. Unused appointments or units after that time are forfeited.

- The number of units used at each appointment is determined by the treating provider’s clinical judgment, up to 50 units. Unused units do not carry over, have no cash value and are not refundable.

- The prize may be redeemed only at Sponsor’s Denison, Texas location. Travel, lodging, meals, and all other costs not expressly stated as part of the prize are the winner’s responsibility.

- The prize is non-transferable, may not be given, sold or assigned to anyone else, and cannot be exchanged for cash or any other product or service, except at Sponsor’s sole discretion.

- Only the prize winner may receive the treatment. Sponsor may require photo identification at each appointment.

9. Medical Eligibility and Prize Restrictions

Botox is a prescription medical treatment. As a condition of receiving the prize, the winner must be at least 18 years old, complete Sponsor’s standard new-patient or returning-patient intake, and attend a consultation with Sponsor’s licensed provider. Whether the winner may receive treatment is determined solely by the provider’s professional judgment, based on the winner’s medical history, current health, medications, allergies, pregnancy or nursing status and other factors. The winner must sign Sponsor’s standard consent forms and follow the provider’s instructions.

If the provider determines that the winner is not a suitable candidate for treatment, or the winner declines to complete the consultation, intake or consent forms, the winner forfeits the prize and the prize will not be awarded, as described in Section 7. Results are not guaranteed. Risks and expected results will be discussed during consultation. Nothing in this Promotion is medical advice.

10. Taxes

The winner is solely responsible for all federal, state and local taxes and any other costs associated with the prize. Because the ARV of the prize is over $600, the winner will be required to provide a valid Social Security number or taxpayer identification number (for example, on IRS Form W-9) before the prize is awarded. An IRS Form 1099 will be issued to the winner for the actual value of the prize received. Please consult a tax professional.

11. Publicity (Optional for the Winner)

Sponsor will not publish the winner’s name, image or any other identifying information unless the winner chooses to allow it. After the winner is verified, Sponsor will offer the winner an optional, separate written authorization to announce the win (for example, by first name and last initial, with or without a photo or testimonial, on Sponsor’s website and social media). Whether to sign is entirely the winner’s choice, and the winner will receive the prize whether or not the authorization is signed. To the extent permitted by law, the winner may revoke the authorization in writing at any time, and Sponsor will stop any further use. Sponsor will never disclose any of the winner’s health information, treatment details or patient status as part of any announcement.

12. General Conditions

Sponsor reserves the right to modify, suspend or terminate the Promotion, in whole or in part, if it is prevented from continuing by any event beyond its reasonable control, including fire, flood, epidemic, labor dispute, act of God, communications or equipment failure, utility or service interruption, or government law, order or regulation, or if fraud, tampering, technical failure or any other cause corrupts or affects the administration, security, fairness, integrity or proper conduct of the Promotion. If the Promotion is terminated, the prize will be awarded in a random drawing from among all non-suspect, eligible entries received before the termination.

No more than the stated number of prizes will be awarded. If, for any reason, more bona fide claims for the prize are made than available, the prize will be awarded in a random drawing from among the persons making purportedly valid claims.

If there is any conflict between these Official Rules and any other Promotion materials, advertising, social posts or statements, these Official Rules control.

13. Conduct

The Promotion Parties are not responsible for the actions of entrants, including attempts to circumvent these Official Rules or interfere with the administration, security, fairness or integrity of the Promotion. The Promotion Parties reserve the right, to the extent permitted by law, to disqualify any individual found or suspected to be tampering with the entry process or the operation of the Promotion, acting in violation of these Official Rules, or acting with intent to annoy, abuse, threaten or harass any other person, and to void all associated entries. ANY ATTEMPT TO DAMAGE OR UNDERMINE THE LEGITIMATE OPERATION OF THE PROMOTION MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAW, AND THE PROMOTION PARTIES RESERVE THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.

14. Limitations of Liability and Releases

To the extent permitted by law, the Promotion Parties assume no responsibility or liability for: (a) lost, late, stolen, undelivered, inaccurate, incomplete, delayed, misdirected, damaged or garbled entries, orders, URLs or emails; (b) incorrect or inaccurate entry information or failed electronic data transmissions; (c) unauthorized access to, or theft, destruction or alteration of, entries; (d) technical malfunctions, failures, errors, omissions, interruptions or delays of any equipment, system, network, payment processor, server, or provider used in the Promotion; (e) unavailability of the internet, the website or the online store; or (f) any injury or damage to an entrant’s or any other person’s computer or device related to participation in the Promotion.

As a condition of participation, and to the extent permitted by law, all entrants release, discharge, indemnify and hold harmless the Promotion Parties and their respective directors, officers, employees, agents, successors and assigns from any and all liability, claims, costs (including attorneys’ fees), losses, damages or actions of any kind for injury, death, or loss to persons or property arising in whole or in part, directly or indirectly, from: (i) participation in any aspect of the Promotion; (ii) the receipt, scheduling or use of the prize, other than the clinical care itself; or (iii) any typographical, human or other error in the printing, offering, selection, operation or announcement of the Promotion or the prize.

This release does not apply to, and nothing in these Official Rules waives or limits, any claim relating to the professional medical care a winner receives. Treatment is governed by Sponsor’s standard consent forms and by applicable law.

15. Governing Law and Disputes

All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrants and Sponsor in connection with the Promotion, are governed by the laws of the State of Texas, without giving effect to any conflict-of-law rules. Any action seeking legal or equitable relief arising out of or relating to the Promotion or these Official Rules will be brought only in the state or federal courts located in or serving Grayson County, Texas, and you irrevocably consent to the personal jurisdiction of those courts.

TO THE EXTENT PERMITTED BY APPLICABLE LAW, ENTRANT AGREES THAT ALL DISPUTES, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THE PROMOTION OR THE PRIZE WILL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND THAT ENTRANT WAIVES THE RIGHT TO SEEK PUNITIVE DAMAGES OR DAMAGES THAT ARE MULTIPLIED OR OTHERWISE INCREASED. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATION OR EXCLUSION OF LIABILITY, SO THE ABOVE MAY NOT APPLY TO YOU. THIS SECTION DOES NOT APPLY TO CLAIMS RELATING TO THE PROFESSIONAL MEDICAL CARE A WINNER RECEIVES.

16. Entry Information and Privacy

Personal information of entrants, including name, address, email, phone number and date of birth, will be used by Sponsor and service providers acting on its behalf only to administer the Promotion, notify the winner, fulfill the prize and comply with applicable laws, and will be handled in accordance with Sponsor’s Privacy Policy at https://absoluteaesthetics.org/privacy-policy. Sponsor will not sell entry information. Entering the Promotion, including by Mail-In Entry, does not sign you up for marketing emails or text messages. You will only receive marketing communications if you separately opt in, and consent to receive marketing is not required to enter or win. Promotion entry information is kept separate from patient medical records, and Sponsor does not use any patient’s health information to run the Promotion. Any health information shared during the prize consultation is handled under Sponsor’s notice of privacy practices and applicable privacy laws, and is not part of the Promotion data.

17. Platform and Payment Processor Disclaimer

This Promotion is in no way sponsored, endorsed, administered by, or associated with Facebook, Instagram, Meta Platforms, Inc., Square, or any other social media or payment platform. You are providing your information to Sponsor and not to any of those platforms. You release those platforms from any liability related to the Promotion.
